If you would like to adopt one of the Forgotten Pet dogs, we ask that you submit an application online (http://www.forgottenpetadvocates.com/redesign/?page_id=4064 – no obligation and a great start).
Although, we ask quite a few questions on this application it is to find out as much information as we can about you and your home. Please answer every question on the application to the best of your ability — failure to do so will slow down the application process. If you have difficulty with the on-line application, please email us at forgottenpetadvocates@yahoo.com. Once we receive your application it will be reviewed by one of our volunteer advocates. If there are questions about your application or clarification is needed, you will be contacted by e-mail or telephone.
We will confirm home ownership or contact your landlord, references and your veterinarian (if applicable). Once your application is approved and the meet and greet is a success, we will contact you to schedule a required home visit at a mutually convenient time (in many occasions the delivery of the dog is done at this moment as well).
Please be patient. Our organization is manned completely by volunteers and most of them work full-time. If you aren’t contacted within 48 hours after submitting the application, please contact us at forgottenpetadvocates@yahoo.com requesting status of your application.
Please note that the adoption application and the adoption contract are separate documents. The adoption contract will be signed at the time you take possession of the dog.
Adoption Criteria:
- Applicant must be a responsible adult.
- A secure fenced yard is preferred although not a requirement.
- Renters (apartment, house, condo or town-home) must have the permission of the landlord/management and may be required to show proof pet deposit has been paid.
- Applicant must have the time, energy and financial capability to support a Forgotten Pet dog.
- Adoptions outside the greater Houston area are considered if the applicant has vet history and/or The Forgotten Pet Advocates can arrange a home visit.
The Cost of Adoption: The adoption fee for dogs is $150 (this is not for out-of-state adoptions). All dogs are up-to-date on all vaccinations, spayed/neutered, micro-chipped, and on heartworm preventative when they are adopted. The dogs may have been treated prior to adoption for other medical conditions. We will disclose as much as we know concerning the dogs health and condition. You will also receive vet records on the dog at the time of adoption. It is recommended that you take your dog’s medical records to your vet as soon as possible in order to receive notice of future vaccinations due and to receive heartworm and flea preventative. The adoption fee is to help off-set the cost the vet care that was already provided. Although the adoption fee covers a portion of the cost of normal vetting, much more is spent on long-term dogs and on some that require extensive medical care/surgeries.
The Forgotten Pet Advocates has the right to deny or approve applications without explanation. Approval should not be interpreted to mean the applicant can select any dog. If we feel a specific dog is not a good match, we may decline to adopt that particular dog to you. A tremendous amount of time and effort is used in matching dogs to forever homes and we want the dog as well as you to be happy.
